Dear Employees,

Open Enrollment begins on Thursday, Nov. 15!

A representative of Hylant (our Insurance Broker) will be presenting in Clearwater FL and Grand Rapids, MI.  The informational session will also be available via a LIVE Webinar and will also be recorded for your convenience.  (See below for link and information for webinar)

The open enrollment period to elect your benefits via Paylocity begins on November 15 and ends on November 29.

The 2019 Benefits Guide along with an instructional guide on how to enroll via Web Benefits is attached.  For those of you who desire a more detailed description, Summary Plan Benefit Docs will be made available on our intranet.  More information on that to follow.

Be sure to participate in one of the sessions below.  No registration required.

To better serve you, we have scheduled Open Enrollment Informational Sessions to better equip you in electing your health benefits.

A representative of Hylant, our Insurance Broker, will be presenting in Clearwater FL and Grand Rapids, MI.  The informational session will also be available via webinar and recorded for your convenience.  Information regarding updates to the plans will be provided at the presentations.

Reminder: New Version of the 4506-T and 4506T-EZ Forms

Dear DataVerify Customer:

The IRS has released new versions of the 4506-T and 4506-TEZ forms (Versoin September 2018), which are ready for immediate use. The IRS will begin redacting transcripts January 1st, 2019. This change will require DataVerify to begin stamping a Customer File Number on the 4506-T forms in December to ensure we are still able to complete work in progress when the change takes place.

DataVerify will start rejecting 4506-T requests on December 10, 2018 if they are not submitted on the new version of the form. If you submit an order using the old version of the form, you will not incur any cost and can submit an updated form once rejected. We strongly recommend implementing these changes as soon as possible to mitigate potential issues.
